Diagnostic challenge

A 25-year old female, presented with fever, headache, shaking chills, myalgias and fatigue. These symptoms developed 4 days after returning from a trip to El Salvador. She reported having multiple mosquito bites while she was there. She also complained of photophobia and nausea. She denied rash, cough, sore throat or diarrhea. On physical exam she had a fever of 103.1 F and a pulse of 132/min. Her WBC count was 3.9 K/uL (n = 3.9–11.0 K/uL), (PMNs = 72%, lymphocytes = 9%, monocytes = 19%),
